Yup, it's a Pushead custom and worm babies are Baby Hyakkimaru by Boa Juice from a manga called Dororo. I traded the original black and gray and was bummed for over a year trying to get another one, great toy, three points of articulation. I did get an unpainted Fungus test shot in exchange for it however. Another figure from Boa Juice I think it's called The Chauffeur of Death. Boa Juice IMO make some of the finest vinyl, figure and quality-wise.
